Silly me, thinking of scanning the business cards and all that. They actually have a website!
Website
http://www.liusbootery.com/
Cost
I was a walk-in customer but had to fly off, so my bill included postage to Singapore. Total (pre-paid) was THB8,000. That's roughly US$248. YPMV.
Self-Measurement
If you feel confident about getting your feet and lower legs accurately measured (by someone else, preferably), you can use this form:

Getting There Physically
If for whatever reason you're actually going to be in Bangkok, they're located at:
280-282 Sukhumvit Rd
Bangkok 10110
Tel. +66-2-653-3811
Fax. +66-2-229-4417
E-mail :
liusbootery@yahoo.com
Hours : 10am to 8pm (Mon-Sat)
By public transport, get to the Asok BTS (skytrain station), exit the south side, ignore the big sign saying "Siam Bootery", and head northwest along Sukhumvit Road; it's on the left (sorry, no Google Street View yet). The station has it's own Wiki page!!?? -

The website has pages for contact details and a map on how to get there.
Notes
There were in fact several booteries in Bangkok that I, erm, espied. But my online research yielded favourable reviews of one specific-but-unnamed bootery at Asoke, described as "just get down from the Asoke station", "you can't miss it" and "it's
not Siam bootery". So it's this one. You can show them the link to my Picasaweb album. I'll put up a scan of my receipt ASAP so he can check his references.
I'm currently also sourcing for even cheaper boots in Singapore (Singapore, cheaper than Bangkok? Really?). But red suede is such a bummer.
If you guys are planning on some big group outing to Bangkok, let me know
Trivia
You'd probably guessed by now that "Asok" is the same name as "Ashoka".
